If you need to request records from your prior physicians so we can include them in your file at our office or you would like us to forward copies of your file from our office, please download and complete the Authorization for Release of Medical Information Form below.
Missed Appointment Policy
Approximately one week prior to your appointment, our office will call to remind you of the appointment. We will leave a message if we are unable to reach you. If you are unable to make your appointment, it is your responsibility to call the office and let us know. We will happy to reschedule the appointment.
If you do not come to your appointment and do not call with 24 hours notice to cancel the appointment, you will be charged a $25 missed appointment fee.
New Patient Appointment Policy
We are very happy that you will be joining our practice and look forward to meeting you. Your appointment has been scheduled usually on the hour or the half-hour, as you will see in Patient Gateway. We do require new patients to arrive at the office 15 minutes before the scheduled appointment so that we can process the patient forms that you have completed. This will allow your provider to see you and the subsequent patients in a timely fashion. If you do not arrive 15 minutes before the scheduled appointment, your provider may not be able to see you during this office session and your appointment may have to be rescheduled.
